The Cask of Amontillado† Unexpected Endings In â€Å"The Cask Of Amontillado† there are several themes shown throughout the short story. There are only two main characters in the story, and each shows their way of betrayal. The entire story is based on betrayal, and lies. Fortunado is betrayed by Monresor, who in the end, kills this intoxicated man. Montresor was supposedly betrayed by Fortunado, a story of which we are never told. Finally, we as the reader are betrayed by the narrator, because we are given so few details and logic to back up the story.Fortunado, Montresor, and the reader are betrayed throughout the short story, â€Å"The Cask Of Amontillado†. Throughout the story, Montresor betrays Fortunado. He asks Fortunado to come into his catacombs aware that he would be killing him. He taunts Fortunado by asking him if he want to go back, because he is sick. Montresor does so when saying â€Å"‘Come,’ I said, with decision, ‘we will go b ack; your health is precious†¦Ã¢â‚¬â„¢Ã¢â‚¬  (Poe 535) However, when Fortunado insists he comes to taste the amontillado, he is once again betrayed by Montresor when he drinks more of his wine.He becomes overly intoxicated and once again, is able to be taken advantage of. The entire walk through the catacombs Montresor betrays Fortunado by lying to him and taunting him about being sick. At the end of the story Montresor traps Fortunado by building him into a small shackled space. â€Å"A succession of loud and shrill screams, bursting suddenly from the throat of the chained form, seemed to thrust me back violently† (Poe 536). According to Chad Dyer â€Å"The story is written in the form of confession, its reader learning of Montresour’s vengeful deed fifty years after its execution. † (Dyer).When Montresor kills Fortunando he commits not only a huge crime but a betrayal that is unbelievable to most people. Furtunado is a simpler, less obvious betrayer. He is mentioned in the very beginning cause a â€Å"thousand injuries† (Poe 533) causing Montresor to â€Å"vow upon revenge† (Poe 533). Despite all the wrongdoing of Montresor, Fotunado was the beginning, and cause of it all. â€Å"It must be understood, that neither by word nor deed had I given Fortunado cause to doubt my good will† (Poe 533). This leaves one to suggest that maybe the narrator was telling us a complete lie. The story was written to keep us guessing. Throughout the entire walk through the catacombs, we had no clue what they were truly walking through. We were given vague descriptions of the bodies lining the walls. When he describes building the brick wall to trap Furtunado, he describes it in a very undetailed description. Montresor needs to repress Fortunato to protect his soul from damnation. (Stott, Online) The only way Montresor can protect himself is to get rid of Fortunato. Montresor can repress the aspect of his personality which insults God by burying Fortunato alive.Fortunato, ironically despite his name, faces a very unfortunate fate at the hands of Montresor. Fortunato’s dress is ironic for a man with his stature in society. Fortunato is a man with stature who is â€Å"rich, respected, and admired† (Poe, Online). Fortunato wears a â€Å"tight-fitting parti-striped dress, and his head is surmounted by the conical cap and bells† for the carnival. † (Poe, Online). Instead of the cap representing Christ’s crown of thorns, the cap represents Satan’s role as â€Å"Prince of Fools† (Pittman, Online).Through the sacrifice of Jesus Christ, Christians were able to receive salvation. Through Fortunato’s sacrifice, Montresor will seek salvation. Montresor dresses more like a priest for the carn ival season. â€Å"Montresor’s black roquelaire symbolizes a priest’s black cope worn during a funeral mass. † (Cooney, Online) Montresor dresses for the occasion that he is going to bury Fortunato. Montresor’s attire suggests that the repression of Fortunato will have an important spiritual reward. Through the repression of Fortunato, Montresor will reach salvation.The carnival setting ironically suggests a time for sin. â€Å"The carnival season consists of the last indulgences in the pleasures of the flesh† (Pittman, Online). Fortunato’s sin of abundantly drinking makes Montresor‘s plan easy to carry out. â€Å"The word carnival can be translated in Italian meaning to put away the flesh which demonstrates symbolic irony of Montresor’s intent for Fortunato† (Clendenning, Online). Montresor can make himself free of sin and worthy of salvation by repressing the sinfulness of Fortunato. The catacombs are an ironic symbol o f the way Montresor thinks.The repression begins when Montresor takes Fortunato into his family catacombs with the promise of Amontillado wine. The catacombs are â€Å"lined with human remains† and that suggests generational family troubles (Poe, Online). Fortunato keeps going farther and farther into the catacombs with only his sinful thoughts of being able to drink more wine, not knowing he is moving closer and closer to his fate of being buried alive. The wall symbolizes the finish of the repression of Fortunato. â€Å"Ironically, the wall forms Fortunato’s burial crypt. (Sweet Jr. , Online) Salvation can only happen when sins are repented. Once Fortunato yells â€Å"For the love of God, Montresor! ,† Montresor says â€Å"Yes, . . . for the love of God! † (Poe, Online). Montresor believes that Fortunato’s repression shows his love for God and demonstrates Montresor reaching salvation. The cask symbolizes a confession. (Sweet Jr. , Online) Montresor is unable to achieve the salvation he seeks through the repression of Fortunato. The Amontillado symbolizes Christ’s bl ood in communion. Christ’s blood offers the salvation that Montresor seeks. The Amontillado may lead Fortunato to his burial but it symbolizes the salvation Montresor wants to gain through repression. [5] A pproximately 37,000,000 hectares (91,000,000 acres) worldwide are now farmed organically, representing approximately 0. 9 percent of total world farmland (2009) History Organic farming (of many particular kinds) was the original type of agriculture, and has been practiced for thousands of years. Forest gardening, a fully organic food production system which dates from prehistoric times, is thought to be the world's oldest and most resilient agroecosystem.After the industrial revolution had introduced inorganic methods, some of which were not well developed and had serious side effects, an organic movement began in the mid-1920s in Central Europe through the work of Rudolf Steiner, who created biodynamic agriculture, an early version of organic agriculture. Organic agriculture was independently developed in the 1940s England through the work of Albert Howard as a reaction to agriculture's growing reliance on synthetic fertilizers. Arti? ial fertilizers had been created during the 18t h century, initially with superphosphates and then ammonia-based fertilizers mass-produced using the Haber-Bosch process developed during World War I. These early fertilizers were cheap, powerful, and easy to transport in bulk. Similar advances occurred in chemical pesticides in the 1940s, leading to the decade being referred to as the ‘pesticide era'. Although organic farming is prehistoric in the widest sense, Sir Albert Howard is widely considered to be the â€Å"father of organic farming† in the sense that he was a key founder of the post-industrial-revolution organic movement.Further work was done by J. I. Rodale in the United States, Lady Eve Balfour in the United Kingdom, and many others across the world. The ? rst lectures and publications on organic agriculture stem from Rudolf Steiner, however, whose Lectures on Agriculture were published in 1925. The modern organic movement is a revival movement in the sense that it seeks to restore balance that was lost when technology grew rapidly in the 19th and 20th centuries. Modern organic farming has made up only a fraction of total agricultural output from its beginning until today.Increasing environmental awareness in the general population has transformed the originally supply-driven movement to a demand-driven one. Premium prices and some government subsidies attracted farmers. In the developing world, many G r o u p V I! In other cases, farmers in the developing world have converted for economic reasons Methods Soil management Plants need nitrogen, phosphorus, and potassium, as well as micronutrients and symbiotic relationships with fungi and other organisms to ? urish, but getting enough nitrogen, and particularly synchronization so that plants get enough nitrogen at the right time (when plants need it most), is likely the greatest challenge for organic farmers. C rop rotation and green manure (â€Å"cover crops†) help to provide nitrogen through legumes (more precisely, the Fabaceae family) which ? x nitrogen from the atmosphere through symbiosis with rhizobial bacteria. Intercropping, which is sometimes used for insect and disease control, can also increase soil nutrients, but the competition between the legume and the crop can be problematic and wider spacing between crop rows is required.Crop residues can be ploughed back into the soil, and different plants leave different amounts of nitrogen, potentially aiding synchronization. Organic farmers also use animal manure, certain processed fertilizers such as seed meal and various mineral powders such as rock phosphate and greensand, a naturally occurring form of potash which provides potassium. Together these methods help to control erosion. In some cases pH may need to be amended. Natural pH amendments include lime and sulfur, but in the U. S. ome compounds such as iron sulfate, alum inum sulfate, magnesium sulfate, and soluble boron products are allowed in oganic farming. Mixed farms with both livestock and crops can operate as ley farms, whereby the land gathers fertility through growing nitrogen-? xing forage grasses such as white clover or alfalfa and grows cash crops or cereals when fertility is established. Farms without livestock (â€Å"stockless†) may ? nd it more dif? cult to maintain fertility, and may rely more on external inputs such as imported manure as well as grain legumes and green manures, although grain legumes may ? limited nitrogen because they are harvested. Horticultural farms growing fruits and vegetables which operate in protected conditions are often even more reliant upon external inputs. Biological research on soil and soil organisms has proven bene? cial to organic farming. Varieties of bacteria and fungi break down chemicals, plant matter and animal waste into productive soil nutrients. In turn, they produce bene? ts of healt hier yields and more productive soil for future crops. Fields with less or no manure display signi? antly lower yields, due to decreased soil microbe community, providing a healthier, more arable soil system Weed management Organic weed management promotes weed suppression, rather than weed elimination, by enhancing crop competition and phytotoxic effects on weeds. Organic farmers integrate cultural, biological, mechanical, physical and chemical tactics to manage weeds without synthetic herbicides. Organic standards require rotation of annual crops, meaning that a single crop cannot be grown in the same location without a different, intervening crop.Organic crop rotations frequently include weed-suppressive cover crops and crops with dissimilar life cycles to discourage weeds associated with a particular crop. Organic farmers strive to increase soil organic matter content, which can support microorganisms that destroy common weed seeds. Other cultural practices used to enhance crop competitiveness and reduce weed pressure include selection of competitive crop varieties, high-density planting, tight row spacing, and late planting into warm soil to encourage rapid crop germination.Mechanical and physical weed control practices used on organic farms can be broadly grouped as: Tillage – Turning the soil between crops to incorporate crop residues and soil amendments; remove existing weed growth and prepare a seedbed for planting; G r o u p V I! Some naturally sourced chemicals are allowed for herbicidal use.These include certain formulations of acetic acid (concentrated vinegar), corn gluten meal, and essential oils. A few selective bioherbicides based on f ungal pathogens have also been developed. At this time, however, organic herbicides and bioherbicides play a minor role in the organic weed control toolbox. Weeds can be controlled by grazing. For example, geese have been used successfully to weed a range of organic crops including cotton, strawberries, tobacco, and corn, reviving the practice of keeping cotton patch geese, common in the southern U.S. before the 1950s. Similarly, some rice farmers introduce ducks and ? sh to wet paddy ? elds to eat both weeds and insects. Controlling other organisms Organisms aside from weeds that cause problems on organic farms include arthropods (e. g. , insects, mites), nematodes, fungi and bacteria. Organic farmers use a wide range of Integrated Pest Management practices to prevent pests and diseases. These include, but are not limited to, crop rotation and nutrient management; sanitation to remove pest habitat; provision of habitat for bene? ial organisms; selection of pest-resistant crops and animals; crop protection using physical barriers, such as row covers; and crop diversi? cation through companion planting or establishment of polycultures. Organic farmers often depend on biological pest control, the use of bene? cial organisms to reduce pest populations. Examples of bene? cial insects include minute pirate bugs, big-eyed bugs, and to a lesser extent ladybugs (which tend to ? y away), all of which eat a wide range of pests. Lacewings are also effective, but tend to ? y away. Praying mantis tend to move more slowly and eat less heavily.Parasitoid wasps tend to be effective for their selected prey, but like all small insects can be less effective outdoors because the wind controls their movement. Predatory mites are effective for controlling other mites. When these practices are insuf? cient to prevent or control pests an organic farmer may apply a pesticide. With some exceptions, naturally occurring pesticides are allowed for use on organic farms, and synthetic subst ances are prohibited. Pesticides with different modes of action should be rotated to minimize development of pesticide resistance.Naturally derived insecticides allowed for use on organic farms use include Bacillus thuringiensis (a bacterial toxin), pyrethrum (a chrysanthemum extract), spinosad (a bacterial metabolite), neem (a tree extract) and rotenone (a legume root extract). These are sometimes called green pesticides because they are generally, but not necessarily, safer and more environmentally friendly than synthetic pesticides. Rotenone and pyrethrum are particularly controversial because they work by attacking the nervous system, like most conventional insecticides.Fewer than 10% of organic farmers use these pesticides regularly; one survey found that only 5. 3% of vegetable growers in California use rotenone while 1. 7% use pyrethrum (Lotter 2003:26). Naturally derived fungicides allowed for use on organic farms include the bacteria Bacillus subtilis and Bacillus pumilus; and the fungus Trichoderma harzianum. These are mainly effective for diseases affecting roots. Compost tea contains a mix of bene? cial microbes, which may attack or out-compete certain plant pathogens, but variability among formulations and preparation methods may contribute to inconsistent results or even dangerous growth of toxic microbes in compost teas. Some naturally derived pesticides are not allowed for use on organic farms. These include nicotine sulfate, arsenic, and strychnine. Synthetic pesticides allowed for use on organic arms include insecticidal soaps and horticultural oils for insect management; and Bordeaux mixture, copper hydroxide and sodium bicarbonate for managing fungi. Genetic modi? cation A key characteristic of o rganic farming is the rejection of genetically engineered plants and animals. On October 19, 1998, participants at IFOAM's 12th Scienti? c Conference issued the Mar del Plata Declaration, where more than 600 delegates from over 60 countries voted unanimously to exclude the use of genetically modi? ed organisms in food production and agriculture.Although opposition to the use of any transgenic technologies in organic farming is strong, agricultural researchers Luis Herrera-Estrella and Ariel Alvarez-Morales continue to advocate integration of transgenic technologies into organic farming as the optimal means to sustainable agriculture, particularly in the developing world. [32] Similarly, some organic farmers question the rationale behind the ban on the use of genetically engineered seed because they view this kind of biotechnology consistent with organic principles. Although GMOs are excluded from organic farming, there is concern that the pollen from genetically modi? d crops is inc reasingly penetrating organic and heirloom seed stocks, making it dif? cult, if not impossible, to keep these genomes from entering the organic food supply. International trade restrictions limit the availability GMOs to certain countries. The hazards that genetic modi? cation could pose to the environment are hotly contested Economics The economics of organic farming, a sub? eld of agricultural economics, encompasses the entire process and effects of organic farming in terms of human society, including social costs, opportunity costs, unintended consequences, information asymmetries, and economies of scale.Although the scope of economics is broad, agricultural economics tends to focus on maximizing yields and ef? ciency at the farm level. Economics takes an anthropocentric approach to the value of the natural world: biodiversity, for example, is considered bene? cial only to the extent that it is valued by people and increases pro? ts. Some entities such as the European Union subsi dize organic farming, in large part because these countries want to account for the externalities of reduced water use, reduced water contamination, reduced soil erosion, reduced carbon emissions, increased biodiversity, and assorted other bene? s that result from organic farming. Organic Farming 4 Geographic producer distribution The markets for organic products are strongest in North America and Europe, which as of 2001 are estimated to have $6 and $8 billion respectively of the $20 billion global market (Lotter 2003:6).As of 2007 Australasia has 39% of the total organic farmland, including Australia's 1,180,000 hectares (2,900,000 acres) but 97 percent of this land is sprawling rangeland (2007:35). US sales are 20x as much. (2003). Europe farms 23 percent of global organic farmland (6. 9 million hectares), followed by Latin America with 19 percent (5. 8 million hectares). Asia has 9. 5 percent while North America has 7. 2 percent. Africa has 3 percent. Besides Australia, the countries with the most organic farmland are Argentina (3. 1 million hectares), China (2. 3 million hectares), and the United States (1. million hectares). Much of Argentina's organic farmland is pasture, like that of Australia (2007). Italy, Spain, Germany, Brazil (the world's largest agricultural exporter), Uruguay, and the UK follow the United States in the amount of organic land (2007). Growth Organic farmland by world region (2000-2008) As of 2001, the estimated market value of certi? ed organic products was estimated to be $20 billion. By 2002 this was $23 billion and by 2007 more than $46 billion. The country with the most organic land is Australia with more than 12 million hectares, followed by Argentina, Brazil and the US. In total 32. 2 million hectares were under organic management in 2007. For 1999 11 million hectares of o rganically managed land are reported. As organic farming becomes a major commercial force in agriculture, it is likely to gain increasing impact on national agricultural policies and confront some of the scaling challenges faced by conventional agriculture.Productivity and pro? tability Various studies ? nd that versus conventional agriculture, organic crops yielded 91%, or 95-100%, along with 50% lower expenditure on fertilizer and energy, and 97% less pesticides, or 100% for corn and soybean, consuming less energy and zero pesticides. The results were attributed to lower yields in average and good years but higher yields during drought years. Organic Farming Organic farms withstand severe weather conditions better than conventional farms, sometimes yielding 70-90% more than conventional farms during droughts. Organic farms survive hurricane damage much better, retaining 20 to 40% more topsoil and smaller economic losses at highly signi? cant levels than their neighbors. The word â€Å"like† appears not simply as a conjunction in the middle of a sentence but often has the honor of starting the sentence. So we routinely hear sentences like â€Å"Like it was hot so I went to the pool.† The above examples point to a lazy and slovenly mind, one that sacrifices clarity for convenience.        George Orwell in his essay â€Å"Politics and the English Language† does not consider slang as particularly offensive. He makes this clear when he states that good writing has nothing to do with â€Å"Americanisms† or with proper use of grammar and syntax. One the pivotal point in his essay is that the meaning must decide the words and not the other way around. Language must be used for the sake of clarity of expression and not evade giving a true picture of things.         A new pressure in our times is the need to be politically correct. This pressure is faced by our politicians, companies and even the average person. No one wants to be perceived as rude, insensitive or offensive or more importantly, face litigation for being politically incorrect. This need to be politically correct can go to extremes. Your lab report for this semester will be a write up of your independent research project. This will follow the standard format for a lab report and should include the following sections: Title Introduction Materials and Methods Results Discussion Conclusion References For this course we are giving extra emphasis to the materials and methods section. This section should include sufficient detail to allow others to reproduce your experiments, without being overly descriptive. A guide to writing each section is as follows: TITLE Name the experiment. The title should be descriptive of what you did or what your data showed. A reader should be able to obtain some understanding of the content of your report from the title. In the research world, scientists scan the table of contents of journals to determine if there are any papers relevant to their research that they should read. Therefore the title is important for getting your work recognized. INTRODUCTION Explain why you choose this project, and what you hoped to learn from it. You will be required to research the background information for your project, and present the current state of knowledge for the topic of your research. In addition, you must explain your rationale for choosing this project, clearly state the objective or hypothesis, and predict the outcome of the experiments if the hypothesis holds true. Example: if an independent research project investigated the effect of hand sanitizers on the growth of E. coli, then the introduction should include background information on hand sanitizers (what they are, how they are used, the ingredients that kill bacteria), and background information on E. coli (what it is, why it is a problem). It would also include the experimental hypothesis, e.g. â€Å"Hand sanitizers will be more effective at killing E. coli than soap.† MATERIALS AND METHODS Describe how you conducted your experiments in sufficient detail that someone else could repeat them, WITHOUT excess detail. First and foremost, this section is NOT simply a list of materials and a step-by-step accounting of what you did. You should write your materials and methods in descriptive form, using past tense (describe what you did). Do not include reasoning in your methods – this belongs in the discussion section. As you are doing your experiment, be sure you keep track of what you actually did in your lab notebook, especially any details which vary from the instructions in the lab manual. Points to remember: * Organize this section carefully and logically, place the methods in the order in which you ran them. * Use subheadings that break the text into distinct sections (if warranted). Include only those procedures directly pertaining to the results you plan to present in the paper. * Include complete mathematical formulas if appropriate. * Do not make the common error of mixing some of the Results in this section RESULTS Present your data in such a way that someone could go directly to the results section and understand the results of your experiments. The results section will have a text portion and a portion that contains figures, tables, photographs, graphs, etc., depending on what kind of data you have. In the text of your results section, describe the trends and important points of your data. Point out what it is that you want your reader to come away with. Be sure you refer to the relevant figures and/or tables when you are writing your text. Refer to them as if you were citing them. For example: The purity of the enzyme improved with each step of the purification scheme (Table 1). For the data portion (i.e., figures and tables), there are many ways to present your results, and you should think very carefully about which is the best way. Is it a line graph, a bar graph or histogram, a pie chart, a table, a picture or diagram, or is it some combination of these? Whatever you choose, be sure it clearly shows your results. You want your reader to be able to look at your tables, charts, figures, etc. and know exactly what experiment was done for each one. Pancreatitis: Causes and Symptoms Acute Pancreatitis Introduction Pancreatitis is inflammation of the pancreas. It is widely accepted that it is caused by pancreatic enzymes digesting their own gland. This leads to inflammation of the pancreas. There are two main forms of pancreatitis, acute and chronic. In acute pancreatitis the pancreas can usually heal itself without any lasting changes to function or structure of the gland. If the pancreas heals but then inflammation reoccurs intermittently and causes irreversible changes to structure and function then it is known as chronic pancreatitis (1). Pathophysiology The exact mechanism is not fully understood however it is believed that the initial events take place within the acinar cells of the pancreas. Injury of the acinar cells leads to an inflammatory reaction localized within the cells. If this inflammation is excessive it can lead to a systemic inflammatory response. The inflammatory process can cause systemic effects because of the presence of cytokines, such as bradykinins and phospholipase A. These cytokines may cause vasodilation, increase in vascular permeability, pain, and leukocyte accumulation in the vessel walls all leading to inflammation. Fat necrosis may also occur causing hypocalcaemia; and pancreatic B-cell injury leading to hyperglycemia (2). A marked systemic inflammatory reaction can lead to ‘distant organ damage and multiple organ dysfunction syndrome (MODS) (1). This is the primary cause of morbidity and mortality in acute pancreatitis. The disease progression can be seen in the following three steps: 1) Local inï ¬â€šammation of the pancreas, 2) A generalized inï ¬â€šammatory response, 3) Multi-organ dysfunction (1) When there is injury or disruption of the pancreatic acini pancreatic enzymes namely trypsin, chymotrypsin and elastase leak into the pancreatic tissue. These enzymes become activated and initiate autodigestion and lead to acute pancreatitis. The activated enzymes break down the pancreatic tissue and cell membranes which leads to oedema, and vascular damage which leads to hemorrhage and necrosis. Some patients who have had a severe attack of pancreatitis who survive through the initial event die following a rather minor insult that would not be life threatening normally. It is said that the two hit hypothesis comes in to play here. The initial excessive systemic inflammatory response primes the immune system so that if another event takes place (a small insult in comparison) for example a chest infection, the immune system is overwhelmed leading to an exaggerated inflammatory response which can lead to death (1). History and examination The main presentation of acute pancreatitis is epigastric pain or right upper quadrant pain radiating through to the back. In many patients sitting forward can relieve the pain a little. The patient would usually also complain of nausea, vomiting and fever. It is important to note a history of previous biliary colic and binge alcohol consumption. The patient may also be tachycardic, tachypneic, hypotensive and mildly jaundiced (2). Abdominal tenderness, distension, guarding, and rigidity are quite common as are diminished or absent bowel sounds. If the inflammation should spread to the lungs then basilar rales may be noted on auscultation of the lung. In severe cases Grey Turner or Cullens sign may also be noted (2). Aetiology There are many causes of pancreatitis. The most common causes being habitual chronic alcohol consumption and biliary stones. In western countries including the UK alcohol abuse is the most common cause of acute pancreatitis. A recent study showed that 44% of patients have alcohol as the primary risk factor for acute or chronic pancreatitis (3). Gall stones can cause pancreatitis as they may become wedged in the pancreatic duct or ampulla of Vater and obstruct the pancreatic duct, leading to release of enzymes into the parenchyma. Other less common causes include: injury (e.g. post ERCP), drugs (such as NSAIDS, azathioprine), viruses (e.g. mumps), autoimmune conditions (e.g. SLE), hyperlipidaemia, malignancy and Scorpion and snake bites (4). Investigations to be done if pancreatitis is suspected 1) Serum enzyme levels: Serum amylase in pancreatitis is more than four times the normal value and lipase is twice the normal and this is diagnostic as there is no other source other than the pancreas, but this test is not always available (5). 2) Full blood count, U+E, glucose, CRP: the CRP value is significantly lower in drug-induced acute pancreatitis and a raised bilirubin and serum aminotransferase is suggestive of gall stones. Low serum calcium levels are quite common in acute pancreatitis and hypocalcaemia is also relatively common. 3) Plain erect abdominal x-ray: this is done to exclude other causes of the symptoms such as intestinal obstruction or perforation. 4) Chest x-ray: this can show if there is a rise in one hemidiaphragm, acute respiratory distress syndrome or pleural effusions which can occur in severe cases of acute pancreatitis. 5) CT with contrast enhancement: this can be diagnostic if clinical results were inconclusive. CT may show swelling, fluid collection and change in the density of the gland. 6) Ultrasound: this is useful to see if the pancreas is swollen and if the common bile duct is dilated. It can also detect gallstones (5). Management In mild cases management is on a general medical ward. Analgesia is given to relieve the pain, usually with pethidine. Morphine is not usually used as it can have a spastic effect on the sphincter of Oddi (4). The patient is given intravenous fluids and not allowed to take anything my mouth. If the patient is vomiting severely then a nasogastic tube is considered. Oral fluids and solids can be taken once symptoms have cleared and blood tests are normal. The cause must then be treated, for example if gallstones were the cause then they must be removed. The severity of pancreatitis is determined by the Glasgow score or Ranson criteria which looks at patient demographics, electrolytes and enzyme levels on admission and 48 hours later (see 1) Glasgow prognostic score Ransons criteria * Age >55 years * WBC >15 x109/l * Urea >16mmol/l * Glucose >10mmol/l * pO2 * Albumin * Calcium * LDH >600 units/l * AST/ALT >200 units Present on admission: * Age >55 years * WBC >15 x109/l * Glucose >10mmol/l * LDH >600 units/l * SGOT >250 units/l Developing during first 48 hours: * Haematocrit fall 10% * Urea increase >8mg/dl * Serum Ca * Arterial O2 saturation * Base deficit >4meq/l * Estimated fluid sequestration >600ml In Severe cases the patent is treated in ITU. There is a high chance of multiple organ failure and infected pancreatic necrosis in these patients so if there is evidence to suggest this then intravenous antibiotics should be administered straight away. The patient should be fed via a nasogastric tube and where there are gallstones present and a high probability of a severe attack early ERCP should be done. Local Complications Pancreatic necrosis is likely if the CRP is rising and is confirmed by a CT scan. Infection occurs in 30-70% of cases of necrosis and this trebles the mortality risk. Fluid collections occurs in 30-50% of patients with acute pancreatitis but in most cases resolves spontaneously. Pancreatic abscess, acute pseudocysts and pancreatic ascites can also occur (6). Systemic complications These include pulmonary oedema, pleural effusions and ARDS with regards to the respiratory system and hypovolamenia and shock with regards to the cardiovascular system. Other complications include: disseminated intravascular coagulopathy, renal dysfunction, hypocalcaemia, hypomagnesaemia, hyperglycaemia and GI haemorrhage (6). Summary Acute pancreatitis is a disease in which there is inflammation of the pancreas. Acute abdominal pain and vomiting are the most common symptoms and increased serum concentrations of the enzymes amylase and lipase can confirm the diagnosis. Injury to the pancreas is mild in 80% of patients who recover well without complications. The rest have a more severe disease and present with local and systemic complications. Alcohol abuse and gall stones are the two most common causes of acute pancreatitis in adults and treatment of mild pancreatitis is supportive and more serious disease needs intervention from quite a few members of the multidisciplinary team (5). Improving the understanding of the pathophysiology and better investigation of the disease severity should improve the management and outcome of this compound disease (5). The New Scenes in Hawks The Big Sleep Essay -- Movie Film Essays

  Ã‚  Ã‚  Ã‚   In the film version of The Big Sleep, Howard Hawks invents scenes and characters that do not appear in Raymond Chandler's novel. No rare bookstore trist, no rough and ready female cabdriver, no winking cigarette girl grace the pages of his book; Marlowe and Vivian never talk of horses; and Carmen's always naked. But not in the film. In the film, she wears clothes, Marlowe is a jockey, Vivian is a horse, and all these characters appear. Faulkner, Brackett, and Furthman write these elements into the screenplay. But they do not develop ideas the text does not already suggest. The ideas are there--just evolved into new species that echo the original animal. Hawks had to do it, for the Production Code forbid directors to present any material that was overtly sexual, violent, vulgar or otherwise, profane. Therefore, since the Hays Office regulated what Hawks could present on film, his writers embedded the censored material in new forms. Todd McCarthy explains that, "the writers . . . and director . . . extract[ed] the maximum character and suggestiveness from every situation" (387). In other words, they invented and modified scenes and created characters while Hawks manipulated the mise-en-scene to suggest the forbidden ideas in Chandler's novel.    Three tenets of the Production Code impact the film directly. The Hays Office states as follows:    1. Sadism, homosexuality, incest, etc., should not even be hinted at in motion pictures. 2. The treatment of low, disgusting, unpleasant, though not necessarily evil, subjects should be subject always to the dictate of good taste and a regard for the sensibilities of the audience. 3. Complete nudity is never permitted. This includes nudity in f... ...tes, but he does not present everything. Nor can he, for the Production Code restricts what he can represent on film in 1946. For this reason, Hawks eliminated some ideas--i.e. Geiger's homosexuality, Vivian's violent aggression--entirely. But he kept the one element he felt he could not omit. In the novel, Carmen's nude figure possesses an incalculable amount of energy. Hawks wanted that energy to carry the film. New Consumer Products :: GCSE Business Marketing Coursework

New Consumer Products Every day companies compete by inventing by inventing a new product. Some of these things are very useful and we don’t know how we would live without them. Many of these products don’t have much impact on society and fade out throughout the years. Most of us can think of many examples such as: Crystal Pepsi, slap bracelets, pogs, and backpack purses. As we look back at the products invented in the last 25 years, we wonder what type of new products we will invented in the next 25 years. Of all the new consumer products invented every year, many succeed and many fail, but it’s great looking in from the outside on the new ideas for our future. Who comes up with these ideas! I remember in the late eighties someone invented a torture tool for women, the more I think about it I’m sure a man invented it. It was made for hair removal; I guess you could use it anywhere on the body with hair. It was a mechanical device that yanked hairs from the roots and this supposedly prevented the hair from growing back as fast. Well, I’m here to tell you this thing was a painful mistake in the inventors’ world. Just five seconds of using this mechanism (and a lot of pain) I decided I would let my legs get as hairy as I could stand. I didn’t need the pain of this root-pulling tool. Needless to say, I wasn’t the only consumer with this opinion; this product took a nosedive into history. This didn’t stop companies; every few months there was a new â€Å"tool† for fast, effective hair removal. Of course, none of these have worked so far. To this day the companies are still trying to promot e these products. The companies that gave up the hair removal products started the evolution of pore cleansing strips. Little strips of gluey paper you wet and press on your face. This supposedly removes dirt and oil from the pores in your face. One company put these in stores, and now every company that sells make-up or facial cleansing products has their own version. Thos invention won’t last long because many consumers have realized tape is a faster, cheaper and more effective. While this isn’t painful like the torturous hair removal creation it’s just another product the world can live without.
